Super Kids Day Care

The allegation is related to a DFPS abuse/neglect investigation and found to be deficient. Based on interviews with children and daycare staff, it was determined a caregiver used profane language towards children in care.

The personnel file was not complete for 1 staff person.

Four children's files lacked health statement documentation.

Three children were sitting at a table that had safety straps that were not fastened. This was corrected at inspection when the caregivers fastened the straps.

There was one caregiver with 16 children. The specified age group was 2 years old. The classroom was out of ratio by 5 children. This was corrected at inspection when another caregiver stepped in to assist the classroom putting the group in ratio.

Six children ages 14-22 months were in the main room with other children watching TV. This was corrected at inspection when the TV was turned off.

Several toddlers were observed sitting at the table or in high chairs for a long period with no activity. This was corrected at inspection when the caregivers gave the children books and toys to play with.

Teachers stepped out of the classroom leaving 16 children unattended to prepare breakfast and answer the door. This was corrected at inspection when the caregivers returned to the classroom and allowed another caregiver to answer the door..

There was one caregiver and six infants ages 6 weeks to 12 months in the room at the time of inspection. This was corrected at inspection when a caregiver returned to the room.

The operation had a broken outlet in the infant room, and the outdoor playground walkway was in need of repair.

Out of five caregivers files reviewed, two caregivers did not have current training.

The playground was in need of loose fill.

A dead insect was found in the infant room. A live insect was found under the sink in the schooler room.
