The operation failed to notified CCR that the named director was on leave for three weeks. They also failed to notify CCR the name of the person that meets director qualifications that will be in charge in the director's absence.
The operation has a caregiver present and in charge of children without having a background check submitted on that person. The caregiver was sent home during the inspection.
The operation did not follow the conditions placed on a person's provisionally eligible. The person was placed in a room to be supervised by a person without a background check on file. This was corrected during the inspection when another qualified caregiver was placed in the room.
