Child's physician nor health care professional was contacted when a child in care at the operation turned blue and required CPR.

When a child became ill at the operation and turned blue, EMS was not contacted.

Caregiver did not use good judgment in allowing a child that required CPR to continue on playing, eating snack, and going outside without seeking medical attention. Caregiver did not use good judgment in not notifying the director and parent immediately after performing CPR on a child in care. Director did not use good judgment in not informing a parent immediately when she was informed that a caregiver performed CPR on a child 10 min after her initial call to the parent but instead waited over an hour to call the parent back.

Parent was not notified immediately of an injury that required CPR and medical attention.

Director did not run the operation in compliance when a parent was not notified immediately that CPR was performed on their child and required medical treatment.

Five of six infant feeding schedules were not signed and dated within the last 30 days.

Three of five infants did not have current feeding instructions on file for review.

One infant was observed in a bed with a loose fitting sheet. The sheet was changed during the inspection. A photo was taken of the loose sheet.

The operation failed to notified CCR that the named director was on leave for three weeks. They also failed to notify CCR the name of the person that meets director qualifications that will be in charge in the director's absence.

The operation has a caregiver present and in charge of children without having a background check submitted on that person. The caregiver was sent home during the inspection.

The operation did not follow the conditions placed on a person's provisionally eligible. The person was placed in a room to be supervised by a person without a background check on file. This was corrected during the inspection when another qualified caregiver was placed in the room.
