While doing a walkthrough, the infant room changing table was torn and no longer able to be sanitized. This was corrected when the operation put a roll of wax paper on the table to be used during diaper changes.

As a result, a child was left unattended on the vehicle for over 30 minutes. It was also stated during the course of the investigation that children are sometimes allowed to turn off the electronic child safety alarm. Allowing a child to turn off the safety alarm does not demonstrate good judgment.

A canister of Bleach wipes was on the table that children were playiing at. This was brought into compliance when the caregiver removed them out of children's reach.
There is no documentation of carbon monoxide detector inspections. Operation believes it may be connected to their fire alarm system. Operation has contacted fire alarm company to advise if this inspection was covered in their annual monitoring. Operation will contact me by Friday to advise. Operation will monitor monthly.
The last fire inspection conducted is dated 7/8/2020 and is expired.
