Four out of four child files evaluated did not have a health statement signed by the child's parent or from their pediatrician.

Two children, age 14 and 19 months, were asleep with their bottles provided to them before nap time.

The primary caregiver completed 13 out of 30 annual training hours for the evaluation period of 3/24-2/25. Training topics missing included 6 hours of child development, half hour of management training, food allergy, building safety, hazardous material and controlling the spread of communicable disease.

Parent rights information was not included in the provider's current operational policies.

Two infants (6 mos) did not have an updated feeing instruction on file. The last feeding instruction was completed in February 2024. One child (6 mos) did not have a feeding plan on file at all.

Two children were missing updated vaccination records from their 5 mos vaccination update.

One child was missing a health statement upon enrollment. One child was missing a health statement from the doctor after being in attendance for 12 months.

The provider last validated the people list 3/2/23.

An emergency diagram was not available for review. Provider was shown a sample diagram and two exit routes should be identified was discussed. NOTE: This is a re-cite.

A 5 mos old infant was observed in a bouncy seat on top of a bed surrounded by pillows. The infant was removed immediately.

The provider failed to timely report to the parents of the 4 children in care that the provider does not maintain the required liability insurance coverage.
