A child is placed in a home with foster parents that do not have active background checks. One of the foster parents had background run on 3/17/17, 4/13/18, and 5/19/20 as a foster parent then adoptive parent with Forever Families. The eligibility end date was 4/27/22. There is nothing currently active/eligible for foster parent. The other foster parent had backgrounds run on 3/17/17, 4/13/18, and 7/28/20 as foster parent then adoptive parent with Forever Families. The eligibility end date was 6/14/21 and there is nothing currently active/eligible for this foster parent.
During a sampling inspection, it was observed that a temporary household member was residing in the home without a completed background check.
A child in care under two years old was unable to walk or stand, appeared drowsy, and required hospitalization. The child tested positive for benzodiazepine which is a physical abuse offense by the foster parent. The foster parent could not explain how the child came into contact or ingested the drug benzodiazepine, admitted to taking non-prescribed Vicodin and being asleep which is a neglectful supervision offense by the foster parent.
A foster parent was asleep, and allowed a child under two years of age to be left unsupervised for an unspecified amount of time.
During the course of the investigation it was determined that a foster parent used corporal punishment as a means of discipline.
