During the investigation, there were multiple accounts of a youth in extended care providing care to another child in care with special needs.

During the investigation, it was found that a child was restrained using an inappropriate device to restrict their movement.

During the investigation, both foster parents failed to provide the level of supervision required by a child's service plan.

During the investigation, a child in care was injured due to insufficient supervision being provided by caregivers.

While reviewing records, It was discovered that three of four records did not have supervisory visits documented.

Three of four records did not contain all required information for an admission assessment. One record is missing the reason why the child was placed in care. The other two records are missing the immediate goals, history of trauma, the current developmental educational and behavioral level of functioning, etc.

A child is placed in a home with foster parents that do not have active background checks. One of the foster parents had background run on 3/17/17, 4/13/18, and 5/19/20 as a foster parent then adoptive parent with Forever Families. The eligibility end date was 4/27/22. There is nothing currently active/eligible for foster parent. The other foster parent had backgrounds run on 3/17/17, 4/13/18, and 7/28/20 as foster parent then adoptive parent with Forever Families. The eligibility end date was 6/14/21 and there is nothing currently active/eligible for this foster parent.

During a sampling inspection, it was observed that a temporary household member was residing in the home without a completed background check.

A child in care under two years old was unable to walk or stand, appeared drowsy, and required hospitalization. The child tested positive for benzodiazepine which is a physical abuse offense by the foster parent. The foster parent could not explain how the child came into contact or ingested the drug benzodiazepine, admitted to taking non-prescribed Vicodin and being asleep which is a neglectful supervision offense by the foster parent.

A foster parent was asleep, and allowed a child under two years of age to be left unsupervised for an unspecified amount of time.

During the course of the investigation it was determined that a foster parent used corporal punishment as a means of discipline.

One of four child records reviewed did not have an Initial Service Plan.

Multiple documents had to be requested due to them not being filed in the records.

Immediate medical care was not provided to a child in care who sustained an injury from a dog bite to the face.

The operation failed to notify Licensing of a foster home moving due to the storm 2-15-21 thru 2-19-21 within 24 hours.

Records requested by the investigator were neve received from the CPA.

The operation did not update the foster home screening when Mr. Lopez got married, when Mrs. Lopez moved into the home, or when the family moved into another home.

The foster family moved into someone's home without it being licensed for over 2 months.

The foster home caught fire, was unlivable and was not reported to RCCR.
