A 9 month old infant was observed sitting in a bouncer watching a movie/showing on a tablet.

A crib in the infant napping area had sheets on the mattress that were loose fitting and not snug to the mattress.

A 6 months old infant was combined with children who are 18 months older the infant (2.5 years to 6 years old) when the operation had a total of 13 children. The infant was moved to the infant room during the inspection.

I observed the TV was on when nobody was watching.

I observed the school age children had access to the kitchen to get cups where the knife was placed next to the cups on the counter top.
