Owner was present at the operation today after being notified that she isn't allowed to be present. The owner had to be instructed to leave the facility.

It was observed during today's follow up that the children were playing on an outdoor play area while chickens were walking around the play area. This was corrected at inspection by staff rounding up the chickens and moving the children to another play area.

Based on the information found as part of a DFPS investigation, it is determined that the director responsibilities were not carried out when a child received a serious injury while in care. It is the director's responsibility to ensure that emergency procedures are implemented and followed to comply with minimum standards

The allegation is related to a DFPS abuse/neglect investigation. Based on information received during the investigation conducted by DFPS, it was determined that the operation staff waited 2 hours and 27 minutes to contact the parents regarding an injury that needed immediate medical attention.

Outlet cover in infant classroom is broken and needs to be replaced. Telephone and cords are on the floor in the infant room that are accessible to children and need to be moved.
