Based on the gatherings during the investigation, it was determined there was concrete evidence to support that prohibited punishment (pulling, grabbing, and slapping hand) has taken place at the operation.

The operation was provided with form 2970 due to the citation 746.2415(b). The due date for parent notification was 10/12/2022. The notification 2970 form was sent out on 10/14/2022.

One infant feeding schedule in the infant room was not updated. The last date updated was on 8/30/22.

2 out of 2 infant teachers did not have SIDS/safe sleep training available for review.

In the 2 infant classrooms (0-12M), it was observed there was stuffed animals, stuffed mini animals on pacifiers, and voice speakers inside the crib while an infant was sleeping. First class had a 3M infant napping and second class had a 9M infant napping.

The parent handbook did not provide information about breastfeeding mothers available for review during the visit.

Based on the information gathered during the investigation, it has been determined that child was hurt while in the operation's care. 3 caregivers were not able to report how the child had gotten the injuries on his head.
