10 child records were reviewed during today's inspection. All 10 child records need to someone as an emergency contact.

There is no documentation that the program practiced a fire drill for the months of September of October.

Group leader's employement affidavit needs to be signed and notarized.

Signed and dated statement needs to be provided for site director that center policies have been received.

During today's inspection employee was observed as person in charge and being unsupervised with children that is in violation with conditions of employee's pervional background.

A caregiver was present at the program in direct contact with children without receiving notification of a cleared background.
