Two outlets in the toddler room had the plates loose. Note: This was fixed when the director's husband put a new plate on the outlets and sealed the edges with caulk. In the three year old room: An outlet was loose and halfway broken away from the wall. This exposed a whole behind it. The door to the restroom in the back of the room was observed to have visible dirt and grime on it.

Both infant classrooms observed had diapers and pulls ups in reach of children. This was corrected at inspection when the director removed the items.

Multiple walls in the toddler classroom have peeling and chipped paint.
I observed 9 children sitting at three tables not doing any activities while the rest of the children were working on various table top activities.
In several classrooms, the diaper changing mat was torn. Note: Corrected when the torn mat was replaced.
On the school age playground an airplane that teeters was on the playground. This was corrected at inspection when the air plane was removed from the playground.
In the restroom several bottles of cleaning supplies was observed under the cabinet in which children had access to. A photo was taken and the director removed the cleaning supplies.

Based on the evidence obtained during the DFPS investigation, supervision was not maintained which resulted in child-on-child sexual misconduct. Although the caregiver was in the classroom, the caregiver was not able to intervene or have close enough proximity to a child to ensure their safety.
A parent informed the operation of an incident that contained serious allegations concerns for children in care. The operation did not report the allegations to Child Care Regulation.
Based on the evidence obtained during the DFPS investigation, a caregiver was on their cell phone while children were in care.

I observed a 3 month old infant sleeping in a gliding swing with a head positioned in the glider. The caregiver removed the child from the glider. Photos were taken.
One infant caregiver was caring for 8 infants ages 2 months, 7 months, 8 months, 8 months, 10 months, 12 months, 12 months, and 13 months. The specified age group is 0-11 months. The caregiver was over ratio for infants by caring for 4 more infants than allowed. This was corrected when another caregiver came into the classroom and stayed.
There is no documentation that a drills were conducted in January 2023.
I observed 6 infants sleeping in playyards with blankets. The caregiver removed the blankets from the infants. Photos were taken.
Feeding sheets in the infant room were not updated and signed by the parent monthly.

Two infants were observed in restrictive devices and the caregiver stated they are left in the device for at least 30 minutes. The infants were removed from the restrictive devices.

In the young toddler room, cots did not have space for caregivers to walk between them.

One infant room had five infants 7-10 months being supervised by one caregiver. This was corrected at inspection when one infant was moved.
The toddler classroom which had 9 children under the age of 2 years had the television on.
The toddler classroom caregiver was unaware and did not have information on the childrens ages.
Cups in the infant and older toddler/pre-k room were not labeled. This was corrected at inspection when all cups were labeled with the child's name.
The toddler classroom with a specified age group of 18-23 months had 10 children and 1 caregiver.
