At the time of the inspection, one personnel record did not have required content training, caregiving for children under 24 months training, as well as other training topics. The caregiver lacked 23 hours.

The operation does not have a qualified director who meets education requirements. The director's credentials expired on January 31, 2023.

The outdoor fence does not open leading outside of the center. This fence is labeled as an emergency exit.

During the time of the inspection, the infant room did not have an adult-sized chair.

The Preschool classrooms (two rooms) were not set up in interest centers at the time of the inspection.

At the time of the inspection the surfacing under the teeter tooter was not at least 6 inches.

During the time of the inspection there were holes observed on the playground that could be a tripping hazard.

The operation did not notify licensing prior to adding new space throughout the building.

Ten outlets was missing a safety cover.

A caregiver being counted in the child/caregiver ratio was left in charge of children before completing the required 8 hours of pre-service training.

The operation allowed a person to be present at the operation prior to receiving a background check determination of Eligible, Provisionally Eligible With Conditions or Eligible With Conditions
