A caregiver did not know how old the youngest child in the room was. This was corrected when the director told the caregiver the age of the youngest child.

There were two slides on the toddler playground, The use zone measured from the stairs of the slide to the brick wall was 3.9 feet and from the slide to the fence was 4.5 feet. The small slide had a use zone of 3.2 from the slide to the fence. These items were removed from the playground.
Observed a bucket full of mop water that was in a closet that did not have the ability to be locked. This was corrected when the mop bucket was removed from the closet.
It was found that a classroom was left unsupervised during nap time.

Observed three caregivers counted in ratio and there was not another caregiver in the building. I spoke to the owner and director about this standard. Another person showed up at the operation during my inspection.
Observed 2 caregiver files, the following was missing from the files; One caregivers file was still missing proof of education and there were only 12.5 hours of training completed from November 2022-November 2023. A second file was still missing the Proof of education and Notarized affidavit.
A caregiver did their CPR online and did not complete a hands on portion of the CPR. I spoke to the caregiver about the guidelines.
A child was sitting in a highchair and was not fastened with the safety strap.The child was fastened into the seat during the inspection.
There was an 13 month old infant in the same room as 4 year old children. There was a 35 month gap between the youngest infant and the oldest child in the classroom. This was corrected when the infant was taken into another room.
There were two outlets in the infant room that was uncovered.
