Two children did not have a health statement from a physician.

The director was missing 14 hours of annual training for the previous training year.

One caregiver was missing 6 hours of annual training hours for the previous training year.

Three infants did not have monthly feeding instructions reviewed by parents.

Seven teachers did not have a notarized affidavit.

Two caregivers did not have completed pre-service training.

One caregiver did not have current CPR training that adheres to the guidelines for CPR for a layperson established by the American Heart Association, and consist of a curriculum that incudes use of a CPR manikin and both written and hands-on skill-based instruction, practice, and testing.

The operation did not have proof that a gas inspection had been completed within the last two years. The last gas inspection report that could be located at the operation was 3/18/2022.

Observed the last fire inspection was completed 8/2022 and it has not been completed since.

An infant was observed to be sleeping in the crib wearing a swaddle. This was corrected when the swaddle was removed from the infant. The infants hands/arms were not free and was confined inside the swaddle.

Observed an infant sleeping in a bouncy seat. This was corrected when Licensing asked for the child to removed from the device and placed in a crib.

It was found during an investigation that a caregiver used prohibited punishments on children in care by tightly grabbing their arm and holding a child's arm behind their back.

A caregiver in one classroom stated that they had 7 children in care. When licensing stated that there were 8 children, the caregiver again said that they had 7 children and then said that they just had a child come in and they do have 8 children in care.

During an investigation it was found that a child was able to get ahold of a vape that had fallen out of a caregiver's pocket.

It was found that parents were asked not to enter the building until March 2022. The Covid emergency orders expired in April 2021, which then allowed full access to the building for parents without denial from the operation staff.

Observed a baby lounger in the infant classroom with an infant using the equipment. This was corrected when the lounger was removed from the classroom.

A health inspection was not able to be provided for review.

A gas leak test had not been completed since 10/18/2019. The gas leak test is required every 2 years.

A caregiver used food as a reward for good behavior.

A caregiver was observed to not sanitize the changing diaper area before placing the next child on the changing table. The caregiver removed the child and sanitized the area before continuing with the next diaper change.

During nap time, a caregiver was laying on the floor with the rest of the children. Caregiver got up and positioned itself to have a better view of the entire classroom.

One classroom did not have the emergency evacuation diagram posted in the classroom. This was corrected when the caregiver posted the diagram next to the entrance/exit door.
