During inspection it was observed that a caregiver did not sanitize a changing table after a diaper change.

It was observed during inspection that a caregiver did not wash their hands after diapering a child. It was also observed on video that a caregiver did not wash hands after multiple diaper changes of children.

During the course of an investigation, it was viewed on video footage that a child was grabbed by the arm and taken to the bean bag for time out by a caregiver.

Staff files were reviewed and found that one staff had the on line only version of CPR certification.

During inspection an infant was observed to be asleep in a swing for 5 minutes. The caregivers were observed on video looking at the infant asleep after 3 minutes. The infant was removed and placed in bed after the inspector asked the child to be moved.

During inspection staff files were reviewed and one fille was missing the signed affidavit.

A child got out of a gate on the playground that was not properly latched and was picked up in the parking lot by a caregiver. A group of children were left unsupervised in a classroom when the caregiver left work for the day and did not leave them with another caregiver.

Allergy Alert plans were reviewed for the HASA and two plans were not complete for two children with diagnosed food allergies.

During inspection there was a sanitizing spray within reach of the children in a classroom. This was corrected at inspection.

This standard was reviewed in relation to the self-reported allegations and found to be deficient. The caregiver stepped out of the room leaving one caregiver in charge of 14 two year old children. During this time a child was injured.

During follow up inspection, 2 infants were observed not strapped in the high chair table.

During inspection it was observed that a cleaning/sanitizer bottle was down within reach of the children. The caregiver returned the bottle to a high cabinet during inspection.

The caregiver did not use good judgment when a child was taken by the wrist and forced to walk around with the caregiver while she cleaned tables. The child tried to get away and sit down and the caregiver did not let go of the child.

A follow up was coducted on this day. This standard was re-cited. A caregiver was observed coming in the operation with the class for gym time and did not complete the transition name to face transition form as required by the operation. The caregiver did not use good judgment when this name to face transition was not completed which ensures all of the children are with the class as required and supervision is met. There were 3 opportunities for this to be completed. This was addressed by the operation during inspection.

The caregiver did not complete this before bringing the bleach in the classroom to be used to clean up an accident.

The caregiver did not use good judgment when the caregiver did not hold a child's hand that is a known "runner" which resulted in a child running away from the class and across the street in to the parking lot another business.

During a walk through of the operation, it was observed by licensing two infants in restrictive devices. One of the Two were asleep. The other was in the swing for 15 minutes. The caregiver removed both children.

A caregiver did not properly supervise a group of children, which allowed a child to be left unsupervised outside.

During inspection the training was observed for 10 employees. Seven of the employees had not completed the 6 additional topics as required.

During inspection the 10 children records were reviewed, one record did not have a health statement.
