During the inspection the following was observed: a child was observed walking from the restroom alone back outside without adult supervision, the toddler caregiver was observed walking out the classroom into the hall way while children were inside the classroom napping without adult supervision, and a toddler caregiver was observed to walk outside the classroom into the kitchen area while children were still in the classroom without adult supervision.

During the inspection the following items were located inside the restroom accessible to children in care: Lysol in the shower area, and eye solutions located underneath cabinet, the grass on the playground was overgrown.

The carpet located in the toddler classroom was observed to have dark stains through out the carpet, food (bread) was observed on the carpet, the window was observed to have dust and stains. The ceiling fan and ceiling in the toddler room was observed to be dirty with dust, and cobwebs. The toys located on the playground were observed to be covered in dirt.
