During the unannounced investigation inspection it was determined that the infant caregivers were not completing or sharing infant daily sheets for infants in care over 12 months of age.
It was determined throughout the course of the investigation that an infant in care was not changed promptly as there was a diaper rash and dried fecal matter observed on the infant's buttocks.
A survey of employee records were evaluated. Two of the five employee records did not have proof of a high school diploma/GED. One employee out of the five did not have a signed affidavit.
