One of the classrooms with a specified age group of two years old had 23 students to two caregivers and was out of ratio by one student. This was corrected when two students were removed and moved to the pre k classroom.
Three out of five of the classrooms observed caregivers could not identify how many children they had or the ages of each child in the classroom. .
None of the medications in the medication bin had a labeled date for when the medication was received. There was also an epinipherine pen that was outside of the box with no date on it.

A 3-year-old child was unsupervised for approximately 1 minute. The child was able to walk out the front door. The child was then seen by a caregiver while waiting outside for her ride.
Based on the information provided during the investigation this standard was found deficient. In effort to redirect, a caregiver pulled a child's chair out from under the child, causing the child to fall and hit his head.
In three rooms during nap cots did not have adequate space for the children and caregivers to walk through. This was corrrected at inspection.
