One caregiver had online CPR training.

Two caregivers did not have signed documentation on file showing preservice or orientation was completed.

This standard was part of a DFPS investigation. It was determined that a caregiver "smacked" a child's hand as a form of discipline.

Children were observed sitting in a table chair without straps. This standard was corrected during the inspection, straps were added to the seats.

Not all of the children that receive transportation services had their parents names and phone numbers available in the transportation binders.

The director did not have current transportation safety training.

A child that is not yet on table foods did not have current feeding instructions on file. The last updated form on file was from 10/10/2022.

The smoke detectors do not get tested monthly.

The carbon monoxide detector does not get tested monthly.

The bus with license plate U29-364's first step has corroded, resulting in a hole.

An infant in the classroom was observed sleeping in a restrictive sleeping device. Per the caregiver, the child had been asleep for forty five minutes.

An individual was present in a classroom, functioning in the role of a caregiver, without a cleared background check.

A child in care sustained an injury that required medical attention, and the incident was not reported to Child Care Regulation.
