This standard was evaluated and found to be a violation of the minimum standards. An employee threatens to call a parent to discipline their child.

This standard was evaluated as part of the investigation. Caregivers were using their phones for personal use while supervising children.

A caregiver did not promptly change a child's diaper.

A three-year-old child was allowed to go to the restroom alone. The restroom was up the hallway (outside of the classroom). And a caregiver did not accompany the child, and no one had visual and or auditory awareness and they were not in close proximity.

Based on information gathered by the DFPS investigator during the investigation, physical abuse occurred. A caregiver picked up a child by the arm and her fingernail scratched the child?s arm. The caregiver also pulled the child?s arm over her head and her feet off of the floor.

This standard was evaluated as part of an investigation. A caregiver picked the child up by one arm, carry the child across the room with the child's arm over her head and her feet off the floor and placed the child in the hallway.

This rule was investigated with regard to an investigation and a deficiency was found. Based on the information gathered, it was determined that a child was left in the classroom unsupervised by a staff person.

One caregiver was caring for eight children. The operation did not received the notice from CBCU prior to letting the caregiver have access to children.
