During the inspection it was observed that a child's nose was wiped while on the playground and the caregiver did not wash their hands afterwards. This was corrected at inspection when the caregiver got some hand sanitizer and used it.

During the investigation inspection it was found that a caregiver that was present during the incident only had 1/24 of their required annual training hours. This was corrected at inspection because the caregiver no longer works for Care 4 Tots.
