Children were observed coming into the building from the playground unsupervised to use the restroom and get drinks of water. The site director inside the building was not notify the children were coming inside.
The operation did not have personnel records on site and could not access them virtually. There was none at the central office to access the records either.
One caregiver was present at the operation and in charge of children prior to the operation received the letter of eligibility.
The operation has two high school students in charge of groups of children. One was present today and observed being in charge of a group of children.
The operation began caring for preK children last school year without notifying CCR.

Two children was stated allergies did not have a food allergy action plan on file for review.
The operation was observed leaving a highschool student alone in charge of a group of children. This was corrected during the inspection when an additional caregiver was added to the group.
A caregiver with a provisional eligible status was observed alone with a group of children.
A caregiver at the operation has been working at the site this school year without a background check on file.
This standard was part of an investigation. It was determined that the only caregiver present and in charge of the children during the time of the incident did not have First Aid training.
A child injured his finger while playing outside and first aid was not provided.
A child was injured while in care and the parent was not notified.
The children were allowed to use an equipment that does not meet Licensing standards, the equipment belong to a public school facility and did not have the proper depth of loose field around the swings and other active play areas, as a result a child got injured when fell from a swing.
