Test data from the manufacturer of the turf/unitary surfacing was not available for review at the time of the inspection.

A 3m old infant was observed in a nap sack that provides restriction across the chest area.

It was stated by a caregiver that the children had been watching a movie in the gym since opening. It had been 2 hours since opening upon my arrival.

I observed a child have access to diaper changing equipment.

2 classrooms were observed out of ratio upon my arrival. The infant classroom had 16 children with 2 caregivers and a specified age group of 12-18m. Another classroom had 14 children with one caregiver and a specified age group of 2 years.

Children were observed eating snacks from the ground in the gym.

Children were observed combined in the gym . 23 children total with a specified age group of 2 years old exceeding the maximum group size of 22.

Children were observed climbing on furniture, sliding and falling because of toys, fighting over toys, running around the classroom. The caregiver was assisting children in an adjoining restroom unable to intervene, causing a risk to children.

Multiple caregivers admitted to yelling at children.

Children are routinely permitted to watch cartoons daily with no relation to activity plans, nor educational value.

The director's position has been vacant at the facility since March 28, 2024.

Caregivers though knowledgeable routinely do not comply with the minimum standards.

Children are routinely combined in the gym area from 6:30am-8:00am.

Screen time is routinely used between the hours of 6:30am- 8:00am.

There were several areas on the walls in the schoolage room where the paint was peeling.

There was no required documentation on the playground surfacing.
