An infant age-child was observed in a crib awake inside a crib for 31 minutes. Note: Child was removed from crib, a rug was brought to the classroom to allow child to move freely around the room.
Staff was not familiar with the ages of four children napping in the multi-purpose room. Staff supervising the toddler and prek children did not know how many children were in care.
One caregiver was supervising a group of eight napping children ages 12mo, 12mo, 14mo, 15mo, 18mo, 18mo, 18mo and 19mo.

Outdoor fence had two loose planks of wood; one of them had nails sticking out. Another area of the fence had three protruding screws. There is a TV in the prek room and another one in the school-age room. Both TV are on top of cabinets and not anchored nor mounted. Note: Fence was fixed during inspection when provider nailed the nails and screws.
One of out six children's records reviewed is missing medical care information, permission to transport, parent/child address, emergency contact address and special care statement.
Two staff files are missing 1-7 hours of pre-service training. A third file is missing copy of ID, employment date and high school diploma/GED. A fourth file is missing orientation statement.
Six staff training record were reviewed. One staff members CPR training expired on 3/31/2022. A second file is missing trainings on transportation, abuse/neglect, safe sleep / shaken baby/ early brain development, hazardous materials, food allergies, medications, emergency preparedness, building and physical premises safety and communicable diseases. Written lesson plans and daily schedules were observed posted in all classsrooms. However some of the caregivers were not aware of the planned activitties and schedule was not being followed. One of the of five groups of children present was observed participating in the planned activity listed in the lesson plan. Infant children do not participate in outdoor play and a child was observed using a tablet to play a game.
Fifty children ages 8mo to 11yo were combined in the school-age classroom for a party. Party lasted over an hour.
