Based on the DFPS investigation information obtained during the investigation there is adequate evidence showing that the director failed to exercise good judgment when she grabbed the child by his arm causing nail imprint injuries. The director did not use an alternate means of redirection.

General Responsibilities for Child-Care Center Personnel ? Based on the DFPS Investigation information obtained there is sufficient evidence to show the caregivers lacked the ability to demonstrate skill to invoke order and redirect children?s inappropriate behavior assigned to their care. Also, there was nothing documented to show that the caregivers called via the walkie-talkie for assistance.

Based on the information obtained during the DFPS investigation, the director did not ensure that qualified staff were placed in the classroom who were able to ensure the classroom was meeting compliance during naptime. Based on the needs and abilities of the children, an additional caregiver should have been placed in the classroom for proper oversight.

Ant killer was on a shelf accessible to children. This was corrected at inspection when the director placed the pesticide on a high shelf where children were unable to access it.

A caregiver in the toddler classroom did not wash her hands after using chemicals to clean the diaper changing pad. Note: When pointed out the caregiver washed their hands.

In the 2 yr old classroom there was a play mat that was torn and as a result could not be properly sanitized. On younger children's playground there was a plastic slide that was cracked. Note: The mat and the slide were both removed during the inspection. Note: Pictures were taken.

In the 2 yr old room there was a electrical outlet accessible to the children that was not covered. Note: the outlet was covered the inspection.

In the older infant classroom the sink was being used to rinse bottles. Note: A picture was taken. Note: New procedures were put in place to wash the bottles in the kitchen.
