During the walk through it was found that the operation has a hazardous piece of playground equipment on the far right of the playground.

Ten children's files were reviewed one file did not have an address for the emergency contact and one did not have emergency contact information at all.
During the walk through a caregiver was not able to provide ages of children to licensing staff when asked. This was corrected when the caregiver updated the roll sheet with the children's current ages for reference.
Ten children's files were reviewed and four files did not have immunization reocrds and they were not school aged children.
This standard was evaluated and found to be deficient due to the operations van window being broken, covered with a trash bag.
During the walk through I observed the unlabeled bottles. This was corrected when the caregiver labeled the bottles.

While reviewing childrens records it was observed that five children's reords were missing the admission date, parent signatures and dates through the admission forms.
While reviewing personnel files it was found that the director and 3 caregivers were missing training hours and including abuse/neglect training.
During the walk through I observed a caregiver change a diaper and the caregiver did not wash her hands. The inspector mentioned handwashing to the caregiver. Then the caregiver washed her hands.
It was found that one caregiver had a pending background check while caring for 17 toddlers during naptime. This was corrected when the caregiver was asked to leave the operation.
Three infants of the ages 4 months, 5 months, and 11 months old were found asleep in restrictive devices during a walkthrough of the infant room. This was corrected when all three infants were removed from the restrictive device and placed in their cribs to sleep.
There is no loose-fill or unitary surfacing under the rocking school bus in the outside play area.
In the toddler room, the two-slide indoor play structure is not installed over unitary shock absorbing surfacing. The two-slide play structure has a large rug underneath the equipment.
The playground with swings did not have a depth of 6 inches of loose-fill surfacing.
A 6 month infant was asleep in a crib while propped up with a boppy pillow. There was no Sleep Execption Form for the infant. This was corrected when the pillow was removed.
In the infant room 2, there were 4 bottles with no name label on them. This was corrected when all the bottles were relabeled during the inspection.
It was observed that the television in the school age room is not anchored.
Caregiver in school age room did not know how many children caregiver was responsible for.
During the walkthrough it was found that one child was confined to a high chair to allow the child to sleep. Note: Picture was taken and child was removed from the high chair and placed in nap mat.
On the pre school playground the wood barriers for the surfacing material were separating and as a result created a tripping hazard.
On the pre school age playground there was a trapeze bar with exercise rings on the play structure.
It was found that the child safety alarm was not installed in the vehicle used for transporting. The vehicle was purchased 2020 when the building opened.
During the inspection it was found that the fire inspection expired 08/23/22.
The training certificates were not available for the caregivers and director.
New outdoor climbing equipment with slide was placed over grass and not proper surfacing.

During the follow up inspection it was found that one infant (10 month old) was sleeping inside the crib with the bib on.
