A caregiver stated they had 20 children when I counted it was 13 another caregiver stated they had 20 children when I counted it was 21 children.

Two caregivers is missing annual training, One caregiver is missing five hours of annual training for year 1-18-22 to 1-18-23 and the other caregiver is missing 6 hours for training year 10-1-21 to 10-1-22

Two children did not have a food allergy plan on file.

Four children did not have emergency care authorization in their file.

The caregiver did not show good judgment when they allowed a child to cross the street to go to their parents car that was parked in the parking lot and not in the parent pick up lane.

Five children did not have information on parents signing off on the operational policies.

Five children's files did not have an address for their emergency contact information.

Caregiver stated they had 22 children and when I counted they was 21 children in care.

It was observed that a caregiver with the 1st- 2nd grade had director care of children with an pending background check.

Younger children were able to walk down the hallway to the restrooms alone, they was not a caregiver supervising the child as they walk down the hallway to the restroom or back to the classroom. Children were allowed to walk around to the dismissal area with out an caregiver.

The operation dose not have a site director at the operation, the operational director has turned into the program director and know has other sites.

Four caregivers had unsupervised access to children without having first aid with in their 90 days of hiring.

Four caregivers had unsupervised access to children without having CPR with in their 90 days of hiring.

The operation didn't have documentation of the address of the alternate shelter away from the center.
