Upon arrival, one infant 3 months of age was observed in a car seat sleeping. The infant was observed sleeping for approximately 15 minutes in the car seat. The infant was restrained in the device for over 30 minutes.

One caregiver was observed on two different occasions leaving children unattended. The first incident Licensing Inspector was observed at the front desk when the Toddler caregiver was observed in the hallway. The second incident occurred at 7:56AM when the Toddler caregiver left 15 toddlers with Licensing Inspector in a classroom while retrieving breakfast waffles from the kitchen area.

Two out of three toilets in the school-age classroom was not in working condition. One out of three toilets was covered with a plastic bag.
The caregiver in the toddler classroom was not aware of the ages of the children in which they were responsible for. The caregiver could not specify the youngest, or the oldest child in care.
One individual who is listed as a staff/employee was left in charge to retrieve children from the transportation center. The individual is 16 years of age, and was left in charge while the Director(s) were providing transportation services.
One caregiver was observed in the infant classroom with 6 infants, 2-15 months of age.
A child 5 years of age was left unattended for approximately 12 minutes on a transportation vehicle. Child neglect was portrayed by operation staff by not conducting a name to face check, and ensuring all children evacuated the vehicle safely.
The driver evacuated the vehicle without following the proper procedures to ensure all children were unloaded, and/or accounted for. The child was left on the transportation vehicle unsupervised for approximately 12 minutes.
Based on the information obtained during the investigation, the operation released two children to a relative without checking identification. The incident occurred on 1/24/2022. Once the older sibling was released, the operation then requested to check identification. Reportedly, the operation did not verify the identification card/photo ID with the children admission forms.
Based on the information obtained during the investigation, the operation staff were not able to provide documentation in order to verify the routine route, the departure time, and/or arrival time.
Based on the information obtained, the transportation driver did not have a list of children who was being transported. The transportation driver was not aware of the names of the children in their care, or the name of the child whom was left on the transportation vehicle.
A transportation driver did not check the transportation vehicle prior to entering the daycare facility. A child was left unattended on the transportation vehicle. The driver was not aware that a child had been left unattended.
During the annual monitor inspection records were not available for review, in which the Director then provided to Licensing at a later date. While reviewing records, it was observed that the trainings provided were conducted by the Director, in which the operation had received an administrative penalty on 07/15/2021. The operation also received an administrative penalty on 11/19/2021. The operation Director provided 20-24 hours of training to their staff.
One caregiver observed in the school-age classroom had an inactive background check. Most recent employment validation took place in December of 2021.
One playground area observed did not have an adequate amount of mulch for surfacing. The mulch had dissolved. The assistant stated that the area has not been utilized currently.
The emergency practice form was not avaible for review during the monitor inspection, and/or the Health and Safety Audit. Inspector was not able to observe emergency drill, fire extinguisher, or smoke detectors documentation.
The most recent fire inspection was dated 07/26/2019. The inspection would be invalid as of 12/30/2021.
