Training certificates did not include all of the dates staff attended training.

Based off of the DFPS investigation proper supervision was not maintained when an incident occurred with a group of children.

Based on information received regarding an investigation, a parent was not immediately notified of a situation placing their child at risk. The incident occurred on 3/13/23 and the parent was not notified until 3/27/23.

A caregiver was allowed access to children in care without a background check being submitted.

The required steps are not being followed when bleach/water solution is used as the disinfectant. the following steps must be used: 1. Spray with soap and water 2. Rinse with clear water 3. Spray or soak in a disinfectant solution for at least two minutes. Rinse with cool water items children are likely to place in their mouths. 4. Allow the surface or article to air dry

A 2 month old infant was swaddled in a blanket while napping in a crib. Infants are not allowed to be swaddled in child-care centers.

A caregiver was observed on camera not washing her hands after changing a children's diapers.

I observed children's diapers being changed on camera and their hands were not washed afterwards.

Based on video footage reviewed by the DFPS Child Care Investigator, children are sitting in or buckled to seats for at least two hours during the day at a time.

The operational policy was observed missing the promotion of indoor/outdoor physical activity.

The outdoor play area was observed having trash including plastic bag and face masks on the ground. This was corrected during inspection when the caregiver was observed removing the trash from the play area.

One out of ten children records reviewed was missing the signature for parents/guardian for special care instructions.

Four out f ten children records reviewed were missing information for emergency contact. This was corrected when director contacted parents for get information and filled in.
