During the investigation inspection it was found that the operation was not able to show records that they have been conducting their daily or monthly inspections to ensure the safety of the building and grounds for the children present.
During the inspection it was observed that a caregiver removed their gloves after changing a diaper and did not wash their hands before moving on to the next child.
During the inspection it was observed that the toddler sink was not equipped with soap and the school age toilet was not equipped with toilet paper.
During the inspection it was observed that parents were not being allowed access into the main part of the operation while picking up their children. They were stopped in the front office and their children were brought out to them.
During the inspection it was observed that 4 infants were having screen time, using their caregiver's tablet, sitting in highchairs.
During the inspection it was observed that a child's diaper was changed and the caregiver did not wash their hands afterwards.
During the inspection it was observed that the changing pad was not sanitzed between each diaper change.
During the investigation it was found that the operation had been deficient in supervision when caregivers were unaware of some of the school age children's behaviors and actions.
During the investigation inspection it was found that the operation had a broken toilet lid, with broken porcelain, accessible to the children in care. It was also found that the changing table in the infant room has a broken strap and is unstable, causing the potential of a baby sliding off. In it's current state, it is not suitable for use and presents a safety hazard.
During the investigation it was found that there was a 2-3 day period in February 2025 in which the operation did not have their air conditioner running and this caused the operation to be too hot inside for ideal and/or safe conditions for the children present.
During the inspection dead roaches were observed in the building and garbage was observed on the playground.
During the investigation it was found that the operation is sometimes out of compliance with state required caregiver to child ratios. One caregiver will be left responsible for a higher number of children than they should have by themselves. This has occurred with the infants, toddlers, and school age children at different times in the past month.
During the inspection it was observed that the infants had one caregiver with seven infants present. This classroom was over ratio by two infants. It was also observed that there was one caregiver with 12, two year olds. This classroom was over ratio by one toddler.
During the follow-up inspection it was observed that a classroom with children under the age of two were having screen time on the caregiver's cell phone.
