A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.
Six staff files were reviewed and the following was determined: Two staff files reviewed needed 21 hours of annual training. One caregiver needed training in SIDS, Shaken Baby, and early childhood brain development.
I observed a seven month old sleeping in a bouncer.
One caregiver was observed caring for 17 children. The youngest was 7 months and the oldest was 5 years. The age difference between the youngest and oldest was greater than 18 months.
The director needed 27 hours of annual training.
Eight children's files were reviewed and the following was determined: Four needed emergency contact info One needed school info One needed a health statement Two needed current immunization records.
