I observed 5 children under 8, transported in a bus van without booster seats.

During the walkthrough of the after-school room, I observed a Lysol and several other bottles of cleaning products in an unlocked sink cabinet accessible to children.

1 of 6 staff files evaluated did not have the emergency care authorization filled out and signed. 2 of 6 files did not have the date of admission in the file. This was corrected during the inspection. 1 of 6 files did not have the complete address for the emergency contact and 1 of 6 files did not have the name and address of the child's school.

1 of 12 staff members had first aid/cpr that expired.

A newly purchased vehicle for Transportation at the center does not have an electronic safety alarm installed which is required.

Ten children's files were reviewed. Eight of the files have not been updated in over one year.

Ten children's files were reviewed. One of the files did not have an admission date. Two of the files did not have health providers information listed. One file did not have signed acknowledgment of having received the operational policies. One file did not have the emergency authorization for care completed. One of the files did not have a copy of the most recent immunizations on file.

Eight employee files were reviewed. FIve of the files did not have a documented high school diploma or its equivalent
