The dual smoke/carbon monoxide detector on the second level was not working properly when tested during the inspection.

There was a multiple occupancy swing on the composite play structure.

The step ladder attached to the outdoor play structure was not secured to the base. There were steps missing on the play structure to climb on the the horizontal ladder.

The admission forms used were incomplete, were missing pages, too, and were outdated. The admission forms do not capture the required minimum standards changes of March 2023. Some of the information missing were health statement from the parent, emergency care authorization, special care statement, and the name of the school.

One child's record did not have the updated immunization record for the child's age.
