The step ladder attached to the outdoor play structure was not secured to the base. There were steps missing on the play structure to climb on the the horizontal ladder.
The admission forms used were incomplete, were missing pages, too, and were outdated. The admission forms do not capture the required minimum standards changes of March 2023. Some of the information missing were health statement from the parent, emergency care authorization, special care statement, and the name of the school.
