A three year old child was found unsupervised in a hallway with no caregiver present to provide appropriate supervision.

The floor of the bridge between buildings had a caved in area with cracked flooring and soft subflooring. Upon reinspection, the floor had not been repaired. The crack had worsened, and other areas of flooring on the bridge had begun to crack.

Four diaper changing areas still did not have sinks.

A sippy cup was observed in an infants bed. This was corrected when the caregiver removed the cup.

A 10 month old infant was observed sleeping with a bib around his neck and near his face. This was corrected when a caregiver removed the bib.

Two infants were observed at the large feeding table without straps fastened. This was corrected when the caregiver fastened both children in.

An infant was observed walking around the classroom with a bottle of formula. This was corrected when a conversation was had with the director about removing the bottle and not allowing any other children to walk around with bottles or sippy cups.

A four month old infant was observed in a restrictive device for one period of approximately 18 minutes, and another period of approximately 24 minutes. This was corrected when the infant was removed and a conversation was had with the director.

A ten month old and four month old infant were observed sleeping on ill fitting sheets that were loose enough to present a suffocation hazard.

Both infant sinks were being used for handwashing and dishes/bottles. This was corrected when a conversation was had with the director and caregivers about designating one sink for handwashing only.

There was sufficient evidence to suggest that caregivers are using their personal phones while supervising the children.

There was sufficient evidence to suggest that infants are being left to cry in their cribs when they are upset.

One of the diaper changing pads had cracks, exposing the absorbant material underneath.

The fire extinguishers had not been checked for July. This was corrected when the director checked them and recorded it on the emergency practices page.

Neither the bus nor the van have had an alarm installed since the last citation.

Two of ten children's files did not have complete emergency authorization information.

Neither the bus nor the van currently used to transport chldren were equipped with a child safety alarm.

Two of ten children's files did not have addresses for emergency contacts.

The operation had not performed fire extinguisher checks this year.

The door had a piece of wood sticking out that caused the child to get a deep cut on middle finger. The operation had the maintenance man fix the door.

The director does not have the required 30 hours of annual training. The director is missing training in the required topics of Handling, Storing, and Disposing of Hazardous Materials, Administering Medications, Preventing and Responding to Emergencies due to Food Allergies, Emergency Preparedness, Preventing the Spread of Communicable Diseases, Building and Premises Safety

The Director and two caregivers who transport, as well as the director, could not provide documentation showing that the required two hours of transportation training has been completed.

The staff did not have the required training in the required topics of Handling, Storing, and Disposing of Hazardous Materials, Administering Medications, Preventing and Responding to Emergencies due to Food Allergies, Emergency Preparedness, Preventing the Spread of Communicable Diseases, Building and Premises Safety
