Based on the information received during the investigation. The caregivers had their backs during the incident and did not see the victim run back to the window. Also while attending to the injuries of the child, the caregivers were not giving adequate supervision to the other children.

A three year old child was found unsupervised in a hallway with no caregiver present to provide appropriate supervision.
The floor of the bridge between buildings had a caved in area with cracked flooring and soft subflooring. Upon reinspection, the floor had not been repaired. The crack had worsened, and other areas of flooring on the bridge had begun to crack.
Four diaper changing areas still did not have sinks.
A sippy cup was observed in an infants bed. This was corrected when the caregiver removed the cup.
A 10 month old infant was observed sleeping with a bib around his neck and near his face. This was corrected when a caregiver removed the bib.
Two infants were observed at the large feeding table without straps fastened. This was corrected when the caregiver fastened both children in.

An infant was observed walking around the classroom with a bottle of formula. This was corrected when a conversation was had with the director about removing the bottle and not allowing any other children to walk around with bottles or sippy cups.
A four month old infant was observed in a restrictive device for one period of approximately 18 minutes, and another period of approximately 24 minutes. This was corrected when the infant was removed and a conversation was had with the director.
A ten month old and four month old infant were observed sleeping on ill fitting sheets that were loose enough to present a suffocation hazard.
Both infant sinks were being used for handwashing and dishes/bottles. This was corrected when a conversation was had with the director and caregivers about designating one sink for handwashing only.
