Two vans in use do not have the guide to first aid in their first aid kit.

The after schoolers were playing video games that were not educational. This was corrected when the teacher had the children put up their electronics and had them engage in activities that were educational such as playing at centers.

There are cracks in the floor in the three's, pre-k and after schoolers classrooms. In the 2's classroom there is chipped paint within reach of children. In the infant room where water comes in after severe storms. There are black spots on the base board. Pictures were taken.

The emergency preparedness plan does not address how children who have limited mobility, or who otherwise may need assistance in an emergency, such as children who have mental, visual or hearing impairments.

In the 2's classroom the changing mat had holes in it. A picture was taken. This was corrected during the inspection when the mat was replaced.

A 3 year old was placed in a high chair for 2 minutes per the teacher. This was a form of punishment.

A caregiver tapped a child on the hand to stop them from climbing a shelf.

In the schoolers boy's bathroom is chipped wood on the wall. It had sharp edges. Pictures were taken.

The cots were not labeled to match the sleep chart for the listed children. This was corrected when the caregivers labeled the cots to match the sleep chart and the child sleeping on the cot.

Two out of eight caregiver records were missing annual training hour. Total hours missing is 35.
