Based on the information obtained, there was sufficient evidence to show children are not given snacks due to their behavior in the classroom.
A total of 24 children were observed in the school-age classroom, 5-11 years of age. A total of 6 children were 5 years of age, and 5 children present were 9 years of age. A total of 4 children were 6 years of age. A total of 3 children were 7 years of age. A total of 4 children were 8 years of age. One child was 10 years of age, and one child 11 years of age.
The inspector observed a tear in the diaper changing mat that was being used in the infant classroom. This would prevent it from being non-absorbent. Picture was taken.
During the inspection, the inspector noticed a fire extinguisher in one of the vans used to transport the children but it was discharged. Corrected at inspection when the director placed another fire extinguisher on the vehicle.
An infant was sleeping an a swing with the safety straps secured in the infant room during the inspection. Note: Discussed the risk of infants sleeping in restrictive devices and the caregivers moved the infant to their crib.
The emergency preparedness plan does not address how children will be accounted for during an evacuation.
There were multiple dead roaches in the toddler bathroom that had not been disposed of. Note: The provider disposed of the dead roaches during the inspection.
A review of the Employees Who Left Employment in 2021 data from February 23, 2022 revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022 as required. NOTE: The operation reported the number of employees who left in 2021 on 3/1/2022 after the deadline.
The operation has not had a recent fire inspection conducted by the Fire Marshal. The last inspection was conducted was 6/24/20.
Records were reviewed for 10 children currently enrolled, 3 children did not have an address listed for the emergency contact, 2 children did not have an emergency contact listed, 1 did not have a number listed for the release information. 1 did not have an authorized release. 1 child did not have a health statement signed by a health care professional.
One caregiver in charge of a group of children ages 5-10 yrs old was not aware of the number of children present. When asked the caregiver began to count and stated there were 25 children present. After the caregiver was informed that there were more children present. After recounting the caregiver then stated there were 29 children in care.
7 cribs in the infant room were observed to have loose fitted sheet.
An 18 month was observed sitting in a high chair. The caregivers indicated that the child would not sleep and was trying to wake another and was placed in the high chair. The caregivers stated that children who do not sleep are placed in the high chair.
Based on the information obtained during the investigation, a caregiver did not demonstrate good judgement when they used a spoon to hit an object out of a child's hands.
