The operation was found to not have provider any incident report to a parent of a child that was placed at risk that is required by minimum standards.
During the investigation it was determine that children were placed at risk when they walked out the emergency door on two different occasions and days the operation did not make a report to licensing the first time this happened.
It was determined that a 20 month old child was left alone in a classroom with the lights off when the caregivers took the class to the playground.
The director was not in compliance with ensuring supervision is being maintained with children in care. The operation was previously cited on 8/25/2021 for a child being left unsupervised.
During review of fire safety the operation was found to not have a current fire inspection.
After gathering evidence, it was determined that a child was left unsupervised for about 6 minutes in the classroom while the group was outside.
The operation failed to complete an incident report within 48 hours of when the incident occurred and have parents sign in a timely manner.
The operation failed to notify the parent immediately and completed the notification 5 hours after the incident.
During the investigation it was determined that the operation was out of ratio for the pre-k class. The SAG is 3yo, maximum number of children is 15 with 1 caregiver and the class was numbered at 18 at the time of the incident.
During the walkthrough I observed a bottle with soap and water and one water not labeled. This was corrected when the caregiver placed labels on the bottles.
During review of infants it was found that 8 out of 21 infants present not yet eating table food did not have current feeding instructions as required by minimum standards.
