During the course of the investigation, it was discovered that caregivers cook food at the center. The center is not equipped to cook the food properly.
Fire extinguisher on the bus needed to be service. NOTE: This was corrected during the inspection by replacing the fire extinguisher with a new one.
Two out of ten children files reviewed needed an updated shot record.
Three out of ten children's files reviewed did not have an health statement available for licensing to review.
A written plan was not available to review for the shared space with HISD.
An updated liability insurance was not available for licensing to review during the inspection. The current policy expired 12/3/2023.
Four infants were not on their cribs but on another sleeping equipment.
Screen time was on during sleep time. This was corrected when the caregiver in charge turned the Television off.
Infants were combined with older children. They were separated when one caregiver arrived.
Boys and girls were on their mats during sleep time. During inspection this age group were separated. Boys were separated from girls.
An air vent was observed to be falling and a ceiling tile was missing in an area used to combine all age groups. Both were observed to be repaired with duct taped which appeared to be falling, There were a loose ceiling tile observed to be falling in a bathroom desginated for children.
A child with a diagnosed food allergy did not have an allegry plan of file signed by the parent and medical professional.
In the 19m-3yr old room, A handwashing sink with under cabinet storage was observed to have Lysol and other cleaning supplies accessible to children. In the kitchen area, a bottle of spray labeled "mat sanitizer" was observed to be stored on the same shelf as food. NOTE: There was a child safety lock on the cabinet which was not functioning properly.
Operational policies were reviewed and about to be missing the following information. Promotion of indoor and outdoor physical activity that is : (A) The benefits of physical activity and outdoor play (B) The duration of physical activity at your operation, both indoor and outdoor (C) The type of physical activity (structured and unstructured) that children may engage in at your operation (D) Each setting in which your physical activity program will take place (E) The recommended clothing and footwear that will allow a child to participate freely and safely in physical activities (F) A plan to ensure physical activity occurs on days when extreme weather conditions prohibit or limit outdoor play.
Two broken floor board were observed. The floor boards exposed the concrete underneath and presented a tripping risk for children,
During the investigation inspection it was discoved that parents are not allowed inside the operation during operating hours.
First aid and CPR training was not available for the director.
During the walkthrough there was a tarp covering play equipment that is to be thrown out that is a hazard for children to possible climb on top of the tarp and get injured or to get under the tarp and get trapped.
During the inspection the children were sleeping on mats and there was not enough space to walk between each mat. This was discussed with the Director and the mats were spaced apart.
