There was no documentation of date of test for carbon monoxide detector for the month of june during the inspection.

Thirteen children were observed unsupervised during naptime, when caregiver was observed leaving the room during the inspection.

A bag of potting soil, a bottle of lighter fluid, and a barbecue pit was observed during the inspection accessible to children in the outdoor play area. This was corrected during the inspection when all the items were observed being removed and inaccessible to the children.

Written activity plans were observed missing dates the plan covers. This was corrected during the inspection when caregiver dated the activity plan.

The Emergency evacuation and relocation diagram was observed missing the designated location inside the center where all caregivers and children take shelter from threatening weather.

Multiple bottles and sippy cups were observed in the infant room not properly labeled with infants fist names and initials of last name. This was corrected during the inspection when the bottles and sippy cups were observed properly labeled.

Two infants 6 months and 11 months of age were observed in crib with toys inside of the crib. Caregivers were advised to remove all toys from the cribs.

There were no current ,updated, nor signed by parents written feeding instruction sheets available for review for 2 infants ages 6 months old.

There was no copy of most recent fire inspection report available for review during the inspection.

Fire extinguishers check was not recorded for the months of June, July, and August during the inspection.

One caregiver with a pending background was observed caring for children in the 0-11 month classroom.

Two infants were observed sleeping in rockers. This was corrected at inspection when the caregiver removed the two infants from the rockers.

There was 1 caregiver caring for 12 children with a SAG of 18-23 months, which put the class out of ratio by 3 children.

The was one infant age 6 weeks that was swaddled while sleeping in the crib. This was corrected at inspection when the caregiver removed the blanket from the child.

The caregiver in the 5 year old old classroom did not have control over the children in her care.

Children were observed sitting in table chairs for over 30 minutes without being able to get down. I observed two children trying to climb out of the chairs.

There was 1 caregiver caring for 6 infants with a SAG of 0-11 months. One caregiver was caring for7 children ages 12m-3y with a SAG of 12-17 months, which put the class out of ratio by 2 children.

There were 4 infants age 12-17 months in the same classroom with children 3 years and older.

The caregiver in the 5 year old classroom did not know the correct ages of the children in her care and did not have documentation to show the children information. The caregiver in the 12-17 month class did not know the correct number of children in her care and did not have documentation to show the children information.

Cribs are labeled with numbers but there was not a sleep chart to indicate which crib belongs to which infant.

There was 1 caregiver caring for 7 infants with a SAG of 0-11 months. This was corrected at inspection when another caregiver was placed in the classroom to assist.

There were 2 infants age 11 months that do not walk and do not have their own cribs for napping.

In classroom 3 there were 2 16 month old children in the classroom with a child of 4, 5, and 7 years old. This was corrected at inspection when 16 month olds were moved over to the infant classroom.

Fire drill was not completed in August 2022.

There were 25 children in a classroom with 2 caregivers with a SAG of 2 years old. The class was out of ratio by 3 children. This was corrected when 3 younger children were moved over to infant class.
