Two caregiver have not completed eight clock hours of pre-service training prior to being counted in the child/staff ratio.

The operational policies were reviewed and the following information was missing: 1. Illness and exclusion criteria 2. Suspension and expulsion policy 3. Safe sleep policy for infants under 12 months 4. Hearing and vision screening requirements 5. Transportation services 6. Water activities offered 7. Field trips offered 8. Parent's rights 9. Procedure for parent to discuss concerns with the director 10. Website for parents to review Minimum Standards 11. Emergency Preparedness Plan 12. Staff requirements for annual training in preventing and responding to abuse and neglect, procedures to increase parent and staff awareness regarding abuse and neglect along with prevention techniques, community organizations the center will work with and actions a parent of a child who is a victim of abuse should take to obtain assistance. 13, Staff vaccination policy. 14. Procedures to support inclusive services for children with special care needs. 15. Inform parents the center is in a gang free zone. 16. Benefits of outdoor play, duration of physical activity for all age groups, type of physical activity, setting of physical activity, recommended footwear and the criteria used when weather prevents outdoor activities.

The center does not have a current fire inspection.

Of the eight employee files evaluated, the following information was missing: 1. Two records lacked documentation of how the education requirement was met. 2. Six staff lacked a notarized affidavit. 3. One staff lacked a pre-employment affidavit

Three caregivers have not completed a pediatric CPR training course that meets the required guidelines of the American Heart Association.
