Records were evaluated for ten children enrolled and the following was determined: Two children needed emergency contact information. Three children needed permissions for transportation. Three children needed permissions for field trips. Four children needed permissions for water activities, including if whether the child is able to swim without assistance. One child needed an updated immunization record. Incident reports completed for two children in care were missing information such as the date and parent/director signature.

During this investigation it was determined a caregiver did not demonstrate good judgement by continuously holding a child's hand and wrists as the child was falling to the floor trying to get away causing the child to have a dislocated elbow.
A caregiver in the Two's I classroom was unaware of the group responsible for. The caregiver stated nine children were present, however ten were observed in the classroom.
An eight month old child was observed asleep on a floor mat. The child was originally placed on a boppy pillow and slid onto the floor.
During the follow up inspection an eight month old non-walking infant was observed sleeping on a mat.
One caregiver was observed with five children ages 8 months, 9m, 11m, 11m, and 15 months. The specified age group was 0-11 months. The caregiver was over ratio by one child.
The caregiver in the Toddler I classroom was observed caring for 9 children ages 14m, 16m, 16m, 17m, 17m, 18m, 18m, and 19m, and 2y. The specified age group for the room is 12-17 months. The maximum number of children one caregiver may supervise for this age group is 5. The caregiver was over ratio by four children.
A twelve month old was observed layng down on a mat in the infant classroom. The child was observed drinking from a bottle.
The caregiver in the Pre-K classroom was unaware of the number of children present. The caregiver stated a total of 18 children were present, however 19 were counted in the room.
The caregiver in the Pre-K classroom was observed caring for 19 children ages 3-5 years old. The specified age group for the room was 4 years old. The maximum number of children one caregiver may supervise for this age group is 18. The caregiver was over ratio by one child.
Based on the information obtained, it was determined that the caregiver failed to demonstrate good judgment as she was issued a citation for failure to control speed after an auto accident.
The caregiver in the Infant classroom was observed caring for 5 children ages 3 months, 6m, 7m, 11m, and 11m. The specified age group for the room is 0-11 months. The maximum number of children one caregiver may supervise for this age group is four. The caregiver was over ratio by one child.
During the follow up inspection a three year olf child left the four year old classroom. The child was discovered hiding under a table.
The caregiver in the Toddler I classroom was observed caring for 6 children age 13m, 14m, 15m, 15m, and 16 months. The specified age group for the room is 12-17 months. Tthe maximum number of children one caregiver may supervise for this age group is five. The caregiver was over ratio by one child.
I observed unlabeled bottles in two of the classrooms. Note: This was corrected during inspection.
I observed tears in diaper changing equipment in four classrooms.
The caregiver in the Three's classroom was unaware of the number of children present in the room. The caregiver stated 11 children were present, however nine were signed in.
Attendance sheets were reviewed for two directors. Time sheets were only available for the week of March 28, 2022.
Based on the information obtained, it was determined that supervision was not maintained when a child fell and broke their collarbone on the outdoor playground.
Based on the information obtained, it was determined that a parent was not immediately notified of an injury requiring medical treatment.
The operation did not notify Child Care Regulation of a child injury that required medical care.
