A 2-year-old child's arm was pulled resulting in the child needing medical treatment for nursemaid's elbow. Licensing was not notified.
A 2-year-old received an injury resulting in a pulled elbow or nursemaid's elbow. The child received medical attention the next day.
A caregiver exposed a child to suffering physical injury. A child was observed with bruises that does not logically match the history or explanation given.
During the investigation, it was learned the operation failed to follow the suicide policy by not administering the suicide screening to a child in care during the first week the child returned home from the hospital. The screening was not done until 2 weeks later.

During a walkthrough of the home the investigator observed several areas of concern: Tree branches in large piles in the back yard and front yard, Clutter, a smell and dog hair were everywhere.
During the investigation, it was determined that children in care were being threatened with Corporal Punishment.
Video obtained shows foster parent using a threatening gesture towards the child. The foster parent also used physical force to grab a scanner from a child causing the child to fall and grab their head.
