During the review of the investigation, it was found that the foster parent omitted to giving the medication "nystatin" but did not record each dosage given.

A 2-year-old child's arm was pulled resulting in the child needing medical treatment for nursemaid's elbow. Licensing was not notified.

A 2-year-old received an injury resulting in a pulled elbow or nursemaid's elbow. The child received medical attention the next day.

A caregiver exposed a child to suffering physical injury. A child was observed with bruises that does not logically match the history or explanation given.

During the investigation, it was learned the operation failed to follow the suicide policy by not administering the suicide screening to a child in care during the first week the child returned home from the hospital. The screening was not done until 2 weeks later.

During the course of the investigation, it was determined that a safety plan was not implemented per the required guidelines set out by the investigation and the child was not properly supervised.

In 1 out of 2 child files reviewed the initial service plan was completed late, in addition, in 1 out of 2 child files reviewed the service plan was not signed by all parties involved in the service planning as required per 749.1309(b)(1)(N).

In 1 out of 2 files reviewed the medication record in the child file is not complete and is missing key components required in this subsection of the minimum standards such as proceadures completed, diagnoses/examination results, and recommended follow-ups.

In 1 out of 2 files reviewed there was no notification of the review after completing it.

In one out of the two files all service planning activities were late commencing with the preliminary service plan.

In 1 out of the 2 files reviewed there was no medication dosage documented in the medication log.

The operation does not have written information to provide at admission to the parents explaining the type and frequency of notifications required.

During a walkthrough of the home the investigator observed two mattresses without bedframes and laying directly on the floor.

During a walkthrough of the home the investigator observed several areas of concern: Tree branches in large piles in the back yard and front yard, Clutter, a smell and dog hair were everywhere.

During the investigation, it was determined that children in care were being threatened with Corporal Punishment.

Video obtained shows foster parent using a threatening gesture towards the child. The foster parent also used physical force to grab a scanner from a child causing the child to fall and grab their head.

One child file and one home file did not have all records on site.

One of the two staff member's files reviewed did not have an updated job description for the staff member's current position.

Foster parent allowed visitor in child in care's bedroom for a period of time with the door closed and did not know if door was locked and did not check or open the door.

3 out of the 3 children interviewed advised the foster parent uses a whooping stick. While interviewing the foster parent she admitted to having a whooping stick and only threatens to use it.

Foster parent admitted to allowing a child in care to be alone with another child, when there was a safety plan in place stating the child is not allowed to be alone with other children. This interaction resulted in the child in care being touched inappropriately by the other child.

Foster parent reported yelling did occur on the day of the alleged incident and denied any inappropriate, condescending, or put me down language used when yelling.

The foster parent did not notify the agency of a change in the household composition, when she allowed her niece to live in the home.

A child in care was not properly supervised by their foster parent who allowed a non-approved caregiver to provide supervision for the child. Additionally, the child in care was physically abused by the non-approved caregiver.

The foster parent did not use prudent judgement when she allowed a non-verified family member to assume care for the children in her care resulting in one of the children being injured by the family member.

A relative of a foster parent, who did not have the required background check, was allowed to live in a foster home and provide care for children in care.

A child in care reported being physically disciplined by a caregiver. This was corroborated by the caregiver, another child in the home, and the foster parent for the children through interviews.

Foster parent had placements and did not have an active background check.

A child in care did not have a 72 hour service plan completed.
