Elena's Day Care

The last gas inspection was conducted on 5/7/22.

Infant was sleeping in a play yard with a visible quarter size hole in mesh.

3 dead roaches were left accesible to children in their restroom.

Monthly fire drills were not conducted under the 3 minutes exit time.

1 child's record did not contain a current immunization record. The child is missing the MMR vaccine.

Children in the infant class have not been signed in since 08/05/22. The Director corrected this at inspection. Children in the prek class were not signed in. A caregiver signed the children in at inspection.

2 children's records did not have health statements available for review.

A review of the Employees Who Left Employment in 2021 data from February 23,2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.

Caregivers were missing training in the required topics of Handling, Storing, and Disposing of Hazardous Materials, Administering Medications, Preventing and Responding to Emergencies due to Food Allergies, Emergency Preparedness, Preventing the Spread of Communicable Diseases, and Building and Premises Safety. One caregiver was also missing training in the required topics of Child Maltreatment, and SIDS/Shaken Baby.

One infant bottle was observed in the infant classroom not labeled with the infant s first name and initial of last name. Note: This was corrected at inspection when the bottles were labeled correctly.

One infant who is not ready for table food has not had their infant feeding schedule updated since June 2021.

The director did not have annual training hours in the required topics of Handling, Storing, and Disposing of Hazardous Materials, Administering Medication, and Preventing and Responding to Emergencies due to Food Allergies, Emergency Preparedness, Preventing and controlling spread of communicable diseases and Building and Physical premises safety.
