During the review of records the provider did not maintain updated liability insurance.

During the follow-up inspection the indoors of the home was free of hazards and the outdoors of the operation still had some of the same previous safety hazards that made it unsafe for the children to play outdoors. While the home conditions had improved significantly, the outdoors still had hazards that included yard tools and broken fence pieces.

During the inspection the operation was re-cited for not having all of the correct admission information for the children in care. A child was missing their emergency medical authorization.

During the follow-up inspection it was observed that there was an adult present in the home without a completed background check.

During the inspection it was observed that one of the two emergency exits was blocked with many items and inaccessible for use as an exit.

During the inspection the primary caregiver was unable to show proof of any of the required trainings.

During the inspection it was found that the toilet available for use to the children was not able to flush properly.

During the monitoring inspection it was observed that the children were served dry cereal and pop tarts for breakfast. This meal did not meet USDA food pattern requirements.

During the inspection it was observed that the children were not having outside time daily.

During the inspection it was observed that there was an adult present in the home without a completed background check.

During the monitoring inspection the indoors and outdoors of the operation had many safety hazards that made it unsafe for the children to play. The hazards outside included yard tools, cords, broken fence pieces, open cat foods with metal exposed. The hazards indoors included walls stacked up with items that created a falling hazard.

During the inspection it was found that the first aid kit in the vehicle and the home were missing a thermometer and scissors.

During the inspection it was observed that there was not many toys or activities available to the children in care.

During the inspection it was observed that there was a 3 month old infant in care and there was not a crib or pack n play available for the infant to sleep in.

During the inspection a child under 24 months was observed using hand sanitizer before sitting down to eat.

During the inspection it was found that one of the child admission records reviewed did not have the required medical authorization.

During the inspection it was found that the substitute caregiver was not current on their CPR and First Aid training.

During the inspection the bathroom of home was observed to not be kept clean and sanitized with visible dirt, dried pee, and hair throughout the bathroom.

There were loose boards on the fence in the backyard as well as a missing gate which left a open gap in the fence. There were also yard tools in the backyard such as a lawn mower and edger.

The primary caregiver had not completed the required annual training.

The operation has not practiced a fire drill since 8/2022.

The primary caregiver provides transportation and does not have transportation training.

The Primary Caregiver did not have a active background check on file as the background check had been inactivated in 7/2022. Note: The background was resubmitted during the inspection.
