Based on the Preponderance of evidence found during the DFPS investigation, a caregiver was found to have Physically Abused a child in care.

Based on the evidence found during the DFPS investigation, a caregiver did prohibited discipline when she grabbed a child by the wrist resulting in a serious injury.

A 7 month old infant was placed a sleep inside the crib with the bib on. Note: This was taken off at inspection.

It was found that the health inspection expired 4/19/23. The inspection was last completed on 4/19/22.

Based on DFPS investigation completed, it was found that a child was left unattended outside for 4 minutes. Child had walked to the front of the office to get inside.

Based on DFPS investigation, after reviewing the camera footage it was found that the caregiver grabbed and dragged the child from the playground.

Based on DFPS investigation, it was found that the caregiver did not know how many children she was responsible for, resulting in leaving a child outside unsupervised.

Based on DFPS investigation it was found that the incident report was not completed concerning when a child was not appropriately supervised by a center employee.

Operational policies are lacking information regarding promotions of physical activity.

Activity schedule shows Pre-k are conducting only 60 minutes of moderate to vigorous play.

Caregiver stated that infants do not go outside.
