One caregiver did not have education documentation in their file.

Two children's records were not kept for 3 months after the two children disenrolled on 7/2025 and 8/2025.

Based on the information obtained for the investigation, there is sufficient evidence to suggest that proper parental notification of minor injuries was not maintained.

Based on the findings of a recent DFPS investigation, there was evidence to suggest that corporal punishment is being used in the classroom.

The center did not report the number of employees that left employment in 2024 by the required date of 1/15/25

A caregiver was observed supervising children in a class without a current cpr/first aid license.

3 of the 4 caregivers folders were observed to be incomplete. Cpr/first aid license, discipline and guidance form and current trainings.

Based on the information received, there is sufficient information to suggest that a caregiver had been yelling and calling children names as a form of discipline.

The operation policy states the parents should arrange a time to visit and conferences would need an appointment. It also states that parents would be interrupting the classroom during the class day.

2 of the 6 classrooms observed during walk through caregivers did not know how many children were in care.

Children were observed sleeping at the door when entering the room.

The fire inspection was not available for review during inspection.

A review of the Employees Who Left Employment in 2021 data from February 23, 2022 revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022 as required.

This standard was investigated in regards to a report received and compliance was not met. The center does not have an open door policy that allows parents to visit the center at any time during the hours of operation to observe their child, center's program activities, building, premises, and the equipment without having to secure prior approval.

The director did not receive training in the following health and safety topics for the October 2019-October 2020 training year: 1. Emergency preparedness 2. Preventing communicable diseases including immunizations 3. Administering medications 4. Preventing, controlling and responding to emergencies related to food allergies 5. Understanding building and physical premises safety 6. Handling, storing and disposing of hazardous materials The director will need to receive make-up training hours that will not count towards the current training year.

Of the 10 personnel records evaluated, one received 19 of the 24 clock hours of required training for the September 2109-September 2020 training year. The caregiver will need to receive 5 make-up training hours that will not count towards the current training year,

Caregivers did not receive training in Understanding Early Childhood Brain Development for their respective training year. The caregivers will need to receive make-up training hours that will not count towards the current training year.

Of the 10 personnel records evaluated, all lacked training in the one or more of heal and safety training topics for their respective training year. The caregivers will need to receive make-up training hours that will not count towards the current training year. Caregivers must received annual training in all of the following topics: 1. Emergency preparedness 2. Preventing communicable diseases including immunizations 3. Administering medications 4. Preventing, controlling and responding to emergencies related to food allergies 5. Understanding building and physical premises safety 6. Handling, storing and disposing of hazardous materials

A classroom of 15 children, ages 18 months to 2 years old, were left with one caregiver when the second caregiver left the classroom. The specified age group was 2 years. With a specified age group of 2 years old, one caregiver can only care for 11 children. This was corrected at inspection when the second caregiver returned to the classroom.
