Based on the information obtained from the investigation, there is sufficient evidence to show a three years old child was left unsupervised in another classroom while in care at the operation.

A substitute caregiver did not have the current training certification to reviewed, The last training certification was observed completed in 2020.

The feeding instruction for four infants in care was obseved not current during the inspection. There were no certification date from three of the feeding intruction sheets from the Infant classroom.

The immunization records for two infants was observed not available for reviewed.

A caregiver from Infant classroom did not wash their hands after finished changing diaper for a child before move on to the next child. This was corrected when the caregiver was informed and washed their hands after each diaper changing period.

The admission information for one infant10 months old was not available for reviewed.

One caregiver was missing eight hours of pre-service training certification before they can be counted in ratio.

One child record was not available for review upon request.

The sleeping mats in the Pre-K classroom were observed being torn showing the filling which is no longer waterproof. This was corrected, new nap mats were availble to replace the damaged ones.

Two classrooms were observed being out of ratio during the inspection.

Two bottles were found to not be labeled with the children's names. This was corrected when the caregiver labeled the bottles with the names of the children.

The operation has not had a health inspection conducted.

The children's products certification was not properly posted. This was corrected when the director posted a copy of the children's products certification.

The operation has not gotten their fire inspection conducted.

A copy of the lunch menu was not posted. This was corrected during the inspection when the director posted a copy of the lunch menu.

The operation did not have a copy of the children's activity plans posted. This was corrected when the owner posted copies of all activity plans for all age groups.
