Five out of six feeding instruction sheets were not updated in the infant room.

The PreK playground had a riding toy that had a large missing piece that could have caused an injury to a child. Corrected at inspection. This was corrected when the director removed the riding toy from the playground. 09/05/2024

Four out six training cups were not balled in the older infant classroom. Corrected at inspection. The caregiver labeled the four training cups with the child first name and last name initial. 09/05/2024

A child was sleeping in front of the exit door. Note: Corrected at inspection. The caregiver removed the child and the cot from in front of the exit door to another area in the room. 09/05/2024

The play area had a long cable cord coming out of the ground and it was loose hanging freely. The school age play area soccer goal netting was coming off the goal and could cause an unsafe environment.

A 6 week old infant had a teething bead lanyard attached to the pacifier in the mouth while asleep in the crib. Note: This was corrected at inspection when the asst. director removed it from the infant pacifier while she was asleep.

Two out of six files did not have the abuse and neglect training that is required by minimum standards.

Based on the information gathered during the investigation, there is sufficient evidence that shows the center did not notify the incident to Child Care Licensing that required medical attention by a health professional.

The PreK class and the toddler class had cots in front of the entrance and exit door in their rooms during nap time. Note: This was corrected when the caregiver moved the cots from the entrance and exit doors.
