Director current tranportation training could not be observed. Last transportation traning observed dated for 7/10/2021.

Fire extingusher was observed last serviced in Feb 2021.

Based upon the information gathered during the investigation, evidence was available to support the safety precautions for unloading children was not followed. As a result, a child was left on the van unattended for approximately 90 minutes.

Based upon the information gathered during the investigation, sufficient evidence was available to support a child was neglectfully supervised when left in the transportation vehicle unattended.

Based upon the information gathered during the investigation, sufficient evidence was available to support the operation failed to report that a child was left on the transportation vehicle unsupervised for an extended period of time within the 48-hour time frame. The child was left in the vehicle on 09/30/2022 and the operation reported the incident on 10/03/2022. The incident should have been reported no later than 10/02/2022.

Based on the documentation provided the fire drill has not been practiced since September 11th. The operation has not documented any emergency drills, smoke detector, and/or the carbon monoxide detector check.

Six out of six staff records did not include current training hours. The most recent training certificates for four out of six staff expired in 2019. Staff files were observed, in which the training topics observed did not cover the required topics.

The caregiver of the school-age group had a cell-phone and computer out while the children in care were playing. The caregiver was not able to properly and sufficiently supervise children in their care while conducting reports on their personal computer.

One out of two toilets utilized in the Toddler classroom is in nonworking condition. The toilet/restroom in the infant classroom is utilized as storage. The operation has missing tiles in the activity area, and the children's bathroom.

The operation had no documentation on the cribs utilized. The operation did not have the manufacturers instructions for the cribs utilized

The caregiver in the Toddler classroom was not aware of the ages of the youngest child in care. The caregiver acknowledged that she is not the primary caregiver in the Toddler classroom.
