There are many large items being stored in the active outdoor playspace that should be removed and stored out of reach of children, such as: a punching bag on a tripod for adult, tire rims, a water table filled with rainwater, cooking grill, miscellaneous trash. 1 large plastic outdoor equipment is cracking and has sharp edges.

A marked bottle of bleach water was stored on the children's bathroom sink counter Note: this was corrected at time of inspection when the primary caregiver removed the cleaning product.

Two out of eight children records reviewed were missing a physician's health statement.

One out of eight children records reviewed was missing the phone number for parent(s).

Two out of eight children records were missing emergency contact information.

Three out of eight children records reviewed were missing health care information.
