Observed five children sitting in table chairs without the safety straps fastened. Four of the table chairs children were sitting in were missing the straps. Note: Two of the children were strapped in properly and the other three children were taken out of the table chairs that were missing the straps and placed in high chairs. The Director placed an order for new straps during the inspection.

There is a door to an electrical room in the girls restroom and a door to another electrical room in the boys restrooms that are not locked nor has a safeguard to prevent children from going into the electrical rooms.
The diaper changing pad in the schoolers classroom contains rips/tears in it. The caregiver stated the changing pad is used for a special needs child.
The operation failed to report two employees by January 15th who are no longer employed at the operation. The two employees background checks are currently active and are due for renewal.
The operation is not practicing fire drills monthly. There is no documentation to show a drill was practiced in January 2022.
The operation does not have recent documentation of the building and grounds maintenance checklist on file. The checklist made available was from December 2021.
